Job Details Social Worker LCSW provides clinical supervision to St. Catherine of Siena PROS( Riverhead) staff to improve clinical services and oversight. Performs as a member of the clinical/IDT team, acts as a community liaison, and provides clinical and/or concrete services. They provide ongoing clinical individual and group therapies, conduct intakes and coordinate care for program participants. This person will have excellent interpersonal abilities, and demonstrate a compassionate, professional manner including verbal, written and listening skills. DUTIES/RESPONSIBILITIES: Conducts Intakes as assigned Provides regular Individual and/or Group Clinical Supervision to Clinical Staff at St. Catherine of Siena PROS. Facilitates admission process; responsible for completion of all necessary documentation. Provides individual and group counseling to both individuals and collaterals such as family members when necessary. Provides consultation to other staff to aid in presenting, identifying and treating personal and social problems of individuals. Assists in arranging for the use of the community resources and provides liaison between the individual and the community. Prepares psychosocial summaries upon admission and as required thereafter. Responsible for D/C planning and referrals, linkage to other programs. Assists with the program recertification process and implement program changes as necessary. Responsible for ensuring timely reporting of work-related injuries, via incident/accident reports, to the Human Resources Department within 24 hours. Interrupts and reports suspected abuse to an administrator immediately and conforms to NYS Justice Center Social Service Law including the Code of Conduct. Keeps immediate Leadership apprised of significant issues. Job requires access to the individuals’ entire protected health information (PHI), in accordance with the Health Information Privacy and Accountability Act (HIPAA) regulations. Able to promote positive relationships with coworkers which foster team building Follow directives of program leadership in any emergency situation or in activity deemed necessary Aware of possible occupational exposure to infectious diseases and conducts themselves consistently with all guidelines set forth POSITION REQUIREMENTS AND QUALIFICATIONS: Licensure/Certification: LCSW (Licensed Certified Social Work) required. Valid NYS Driver’s License required. Experience: One (2-3) years experience with the population required Requires a good understanding and ability to incorporate the psychiatric rehabilitation philosophy in working with consumers. Requires knowledge of the principles and methods of social work. Salary Range USD $36.68 - USD $50.97 /Hr. The salary range or rate listed does not include any differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
