**Job Description** **Job Title: Social Worker - Early Childhood** **_Social Work Services at the Mount Sinai Hospital_** The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities. + The Icahn School of Medicine at Mount Sinai Hospital is seeking a licensed clinical or master social worker to serve as a HealthySteps Specialist, in our diverse, multidisciplinary, patient centered medical home at the Cohen Center for Pediatric Primary Care (CCPCC). + The position is full-time, providing opportunities for clinical work, teaching/training, and collaborative research/quality improvement efforts. + The social worker/Healthy Steps Specialist in this new grant-funded position will join the pediatric primary care team at the Cohen Center for Pediatric Comprehensive Care to provide interventions, referrals, and follow-up for families with patients, ages 0-5. + The HealthySteps Specialist builds strong relationships with families and providers to increase the efficiency of the medical system and support team-based comprehensive care. + The HealthySteps Specialist will be part of the very dynamic pediatrics practice comprised of a team of pediatricians, pediatric residents, clinical social workers, nurses, administrators, a child life specialist, and medical assistants who are highly motivated to provide high quality team-based care to patients. + The practice serves a diverse population including a high proportion of children ensured by Medicaid and we value caring for children and families with a diversity, equity, inclusion, access, and belonging lens. **Qualifications** + LMSW or LCSW in the State of New York is required prior to beginning the position. + Experience and knowledge about integrated behavioral healthcare, early childhood growth and development, parent-child relational health, infant and early childhood mental health, and family systems is preferred. + Early child development expertise. + Strong experience and/or interest in team based care and ability to work flexibly and cooperatively with a large multidisciplinary team. + Exceptional engagement, assessment, crisis, and short-term treatment skills. + Comfortable with caseload tracking and reporting of metrics. + Ability to develop a focused practice in a new setting with interest in program planning and development. + Strong collaborative, organizational/time management skills and flexibility. + Bilingual (English/Spanish) _preferred._ + LCSW _preferred._ + Internal candidates: Minimum of one year in current position. Non-Bargaining Unit, 186 - Social Services - MSH, Mount Sinai Hospital **Responsibilities** + Promoting HealthySteps within the practice to families and providers. + Conducting team-based well-child visits before, during, or following the primary care provider visits with a focus on monitoring development, social-emotional functioning, and relational health, and providing anticipatory guidance. + Conducting consultations on a short-term basis for needs regarding development and/or behavioral concerns such as sleep, positive parenting, picky eating, etc. + Maintaining a child development support line and respond to and tracks call requests within the designated response time. + Managing and tracking referrals placed, completed, and resulting recommendations. + Participating in reflective supervision meetings. + May conduct home visits and may accompany families to key medical, specialty, and community agency appointments as needed. + Maintaining data needed for the Health Steps program as well as required documentation in the electronic health record. + May facilitate parent/caregiver groups. **About Us** **Strength Through Diversity** The Mount Sinai Health System believes that diversity, equity, and inclusion are key drivers for excellence.
