Description
Support and advance the mission of service to patients and their families, as part of the Palliative Care team, through the advanced clinical practice of independent professional social work. The primary responsibilities are to coordinate, evaluate, educate, case manage and provide direct care to meet the psychosocial needs of palliative care patients and their families. The goal is to assure continuity of their health care as well as maintain and/or enhance their quality of life.
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
· Licensed by the State of Iowa as a Licensed Independent Social Worker (LISW).
· Master’s degree in social work required.
· Two (2) years of experience with patients who have chronic illness or disease preferred.
· Proof of completion of Mandatory Reporter abuse training specific to population served within three (3) months of hire.
· Demonstrates competence in providing case management services.
· Demonstrates skill in conducting clinical interviews.
· Ability to understand medical terminology and communicate effectively with persons of diverse backgrounds and ages.
· Knowledgeable regarding clinical interviewing techniques.
· Knowledgeable regarding social work principals, standards and ethics.
· Basic understanding of Medicare and Medicaid regulations, various serious disease processes, and child/dependent adult laws and regulations are desired.
