Sheltering Arms Institute is seeking a Social Worker (LCSW) to join our team in a two year grant funded position. This is a 2 year grant funded position with potential for more. The social worker will support the Commonwealth Neurotrauma Initiative Grant (CNI) in providing family and caregiver support for traumatic brain injury and traumatic spinal cord injury population in the inpatient setting and across the continuum of care for up to 6 months post discharge. Works in collaboration with the transdisciplinary team to assist individuals, groups and families coping with psychological, cognitive and physical disabilities through therapeutic interventions, support groups, referrals to appropriate community resources and the education of available government entitlement programs.
Some responsibilities will include the following:
Implement, support and facilitate group and 1:1 caregiver support services Facilitate 2 inpatient caregiver support groups monthly for Brain Injury and Spinal Cord Injury Refers caregivers and patients to community resources as needed Use acquired data to create ROI for LCSW reimbursement for operationalizing future Collaborates with inpatient rehabilitation providers to coordinate care for complex psychosocial concerns to create cohesive plan for emotional-behavioral wellness after dischargeQualifications and Skills
Previous experience working with patients and families with brain and spinal cord injuries or a physical rehabilitation setting preferred. Previous experience in social work in a health care setting preferred. Experience leading support groups and coordinating community resources and referrals preferred. Excellent listening and communication skills.Education/Certifications
