Social Worker-Joslin Pediatric Clinic
Job Summary: The medical Social Worker will work at the Upstate Joslin Pediatric Center. The Social Worker will provide comprehensive psycho-social assessment, management, and support to at risk patients and families and will work closely with the providers, registered dietitians, and day staff to provide high quality stabilization and treatment for diabetic pediatric patients. The Social Worker's primary responsibility is to function as a consultation and referral service to address the psychosocial needs of the patient population served by both inpatient and outpatient areas of Upstate Medical University. The Social Worker contributes to the achievement of optimum health, satisfaction, and cost-effective care by addressing the social determinants of health which may impede care. Minimum Qualifications: MSW from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit. Preferred Qualifications: Work Days: Monday-Friday 8:30-5. Occasional on-call/holiday coverage on rotation, based on operational needs.
