Social Worker-LCSW

JOB SUMMARY: 

Provides psychosocial assessment, screening, counseling, and crisis intervention services based on established high risk criteria and on consultation basis to patients and families, utilizing a variety of treatment modalities, which include but are not limited to individual, family, group and specialized counseling.  Works collaboratively with the multidisciplinary team to determine services that will be needed to assist patients and their families in reaching their goals of care, and that promote positive outcomes.

JOB RESPONSIBILITIES:

Able to deliver information to patient/family in a direct sensitive manner. 

Communicates routinely with supervisor and others regarding cases, objectives, and results as well as any issues that may impact staff, department or work flow.

Work produced meets quality requirements of accuracy, thoroughness, and efficiency.

Able to handle conflict. Develops creative interventions to resolve conflict. Able to defuse situations to produce a positive outcomes.

Handles all computer system access, patient information, UM and quality reviews and reports according to departmental/hospital policy and regulatory/state requirements for patient confidentiality.

Initial assessment and re-assessment are documented according to departmental policy guidelines.

Effective, professional and appropriate communication with peers, staff, physicians, patients, families and community agencies/facilities.

Coordinates care by using the appropriate resources; formulates, coordinates, communicates and documents clear and concise transitional planning efforts, including final dispositions in Electronic Medical Record as well as additional software programs utilized (for example: MIDAS, Care Port etc.)

Strong working knowledge of community and financial resources. Able to educate and counsel patients/families regarding entitlements and other financial resources.

Identifies and reports domestic violence and suspected abuse to appropriate protective service agencies.

Perform Suicide Risk Assessment when identified.

 

EDUCTAIONAL REQUIREMENTS:

Master’s Degree in Social Work from an accredited school of social work.

 

LICENSE REQUIREMENTS:

Current New York State License Masters Social Worker (LMSW).

 

EXPERIENCE:

1-3 years of relevant experience required. Experience in specialty areas (for example: Emergency Room, dialysis, rehabilitation, substance/ETOH, home care, oncology, and hospice preferred). Discharge/Transitional Planning De-escalation skills to manage crisis situation Knowledge of Community Resource and Entitlement programs and eligibility Provide psychological, emotional support and counseling Identify, assess, and report suspected abuse cases.

Additional requirements for job that will be provided upon hire, if not already obtained:

SBIRT Certification NYS DOH Screen Certification CPI Certification- Non-Violent Crisis Prevention

 

 

The pay for this position ranges from $36.56 – 51.19 per hour/per year.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
