Social Worker LCSW - Hospice

Schedule: Full-time, Day shift

The Clinical Social Worker, LCSW, is an integral member of the palliative care consultation team. The LCSW provides biopsychosocial assessment, treatment planning, and implementation of social work services to patients and families who have been referred to palliative care. The LCSW utilizes specialized knowledge and clinical skillsets in end-of-life and palliative care to address needs of vulnerable populations and those with serious illness. In addition to addressing barriers to coping with illness or injury, the LCSW has a specialized role with regard to palliative care assessment and intervention of anticipatory grief, loss, complicated mourning, pain management, suicide risk, and other complex mental health issues associated with chronic and progressive illness. The LCSW demonstrates proficiency in palliative care, end-of-life, and/or hospice care.

Required qualifications:

Master's Degree in Social Work Upon hire: California Clinical Social Worker License Or Upon hire: California Marriage and Family Therapist License  2 years of experience in hospital, medical outpatient clinic, hospice, palliative care, oncology setting, or other related healthcare setting.  Computer experience as needed for the job.

Preferred qualifications:

Experience in hospice, palliative care, grief and loss.
