Provides psychosocial assessment and delivery of counseling, social work and emotional support services to terminally-ill patients, and/or palliative services to patients and their families.  Works as a team member to identify problems and assist patient/family members in exploring realistic options, decision making and problem solving.

Required qualifications:

Master's Degree in Social Work, Psychology or Counseling. Graduate from an accredited school preferred or Ph.D. in Social Work, Psychology or Counseling.  Graduate from an accredited school preferred.     Coursework/Training: Training in family counseling and case management.    Upon hire: California Clinical Social Worker License or California Marriage and Family Therapist License  1 year Experience in a health care setting or related field.
