Provides psychosocial assessment and delivery of counseling, social work and emotional support services to terminally-ill patients, and/or palliative services to patients and their families. Works as a team member to identify problems and assist patient/family members in exploring realistic options, decision making and problem solving.

Master's Degree Social Work, Psychology or Counseling. Graduate from an accredited school or Ph.D. Social Work, Psychology or Counseling. Graduate from an accredited school Coursework/Training: Training in family counseling and case management.
