Provides psychosocial assessment and delivery of counseling, social work and emotional support services to terminally-ill patients, and/or palliative services to patients and their families.  Works as a team member to identify problems and assist patient/family members in exploring realistic options, decision making and problem solving.

Required qualifications:

Master's Degree in Social Work from an accredited school. Graduate from an accredited school preferred or Ph.D. in Social Work Coursework/Training: Training in case management.    Upon hire: California Clinical Social Worker License  1 year Experience in a health care setting or related field.
