Overview M Health Fairview has an immediate opening for Social Worker/ Licensed Alcohol Drug Counselor to join the Solid Organ Transplant team based in Minnesapolis, Mn. This is a full time, 80 hours every two weeks, benefit eligible position located on the University of Minnesota Campus. Responsibilities Job Description The incumbent provides a wide range of clinical social work services and interventions to assist individuals, families, groups or communities to restore, enhance, or protect their capacity for productive social functioning and coping with the consequences of their illness and health care status. They complete and review mental health assessments at time of transplant candidacy evaluation as a member of the transplant interdisciplinary team to determining candidacy, and will determine clinical interventions, initiate crisis evaluation, counseling and psychosocial assessment of patients as well as assisting patients with short-term counseling, facilitating therapy and providing education for patients and their families and facilitating treatment planning while managing cases and planning following medical crisis during transplant candidacy evaluation. Additionally, they will continue to assess the patient during the transplant waitlist period, during admission for transplantation and post-transplant. Incumbent also ensures appropriate decision makers are informed of and involved in treatment planning. The individual is able to assume independent practice and takes a leadership role in patient care, program, and professional issues that they confront in their daily practice with minimal supervisory guidance. Demonstrates a range of social work interventions that include patient assessment in the transplant candidacy and treatment planning process, case management, counseling, crisis intervention, and advocacy in assisting patients and families reach their goals. Additionally, develops an individualized treatment plan based on age, social and cultural considerations, clinical diagnosis, level of functioning and other relevant needs, partnering with a multi-disciplinary team Demonstrates clinical competence in conducting individual therapy, group therapy, family therapy and psycho-education groups Demonstrates the ability to advance evidence-based practice by utilizing standards, guidelines, and pathways for care delivery. Incorporates data and information into practice to continually improve care and practice so that patient outcomes are enhanced. Qualifications Required Master’s degree in psychology, social work, nursing or closely related field One of the following current Minnesota licenses: Licensed Independent Clinical Social Worker (LICSW), Licensed Psychologist (LP), Licensed Professional Clinical Counselor (LPCC), or Licensed Marriage and Family Therapist (LMFT) Licensed Alcohol and Drug Counselor (LADC) Basic Life Support certification prior to end of orientation. Preferred 2 years of supervised therapy experience in mental health/substance use disorders including assessment, individual, group and family therapy.
