Social Worker - MSW ($5000 incentive)
Overview Pediatric Kidney Social Work Position: This is a full time 1.0FTE (80 hours/2 weeks) benefit eligible Social Work position supporting the Pediatric transplant program and peritoneal dialysis patients at the University of Minnesota Masonic Children's Hospital. The pediatric kidney transplant social worker supports children and their families throughout the transplant process. They assist with emotional and psychological needs, coordinate with healthcare teams, help navigate insurance and financial issues, and provide resources for education and coping strategies. Their ultimate goal is to ensure a smooth transition and ongoing support for the child's well-being. This is an onsite position and will be a combination of outpatient clinic and inpatient hospital work. This position will have an every 6th weekend rotation with two weekdays off in that pay period. This position is offering a $5000 incentive! Responsibilities Job Description Reasons for SW Interventions may Include but are not limited to: New diagnosis or change in treatment plan Child protection issue/security concern    Emotional needs (adjustment, coping, emotional support)   Community resource needs Mental health assessment and/or crisis intervention Financial/insurance/legal concerns    Transportation concerns   Housing/lodging concerns   Caregiver support and/or concerns Transplant and Dialysis Eval Family Care Conferences Role Duties: Support the patient and family’s coping and emotional needs.   Promote adaptation to illness and treatment    Help families navigate through the medical system, bridging the connections between the family and the nephrology interdisciplinary team.   Provide counseling to individuals, siblings, couples, and families    Help with problem solving, advocacy and communication about the patient’s plan of care   Help with school issues (arranging for home and hospital teaching, advocating for IEP and 504 educational plans, and guidance with school re-entry)   Help identify hospital and community-based resources: emotional, supportive and/or financial   Qualifications LGSW/LICSW Previous healthcare experience preferred.
