Social Worker - MSW ($5000 incentive)

We have a new 1.0 FTE (80 hours/2 weeks) benefits eligible position supporting the Maternal Child Health Service line at the University of Minnesota Masonic Children's Hospital. This position is offering a $5000 incentive! Areas of service include Labor and Delivery, Post Partum, Ante-Partum and Newborn Family Care and our Level 4 NICU. There is an every 6th weekend rotation for the Maternal Child Health unit with two week days off within that pay period. This position also offers Clinical Supervision. Responsibilities Job Description The Maternal Child Health Social Worker Provides a range of social work interventions that include, case management, counseling, crisis intervention, and advocacy in assisting patients and families reach their goals. Identifies and recognizes rapidly changing situations and quickly intervenes to prevent or resolve the emotional crisis. Plans and implements treatment and transition care plans based upon a psychosocial assessment that is integrated into an interdisciplinary care plan. Arranges, mobilizes, utilizes, and may coordinate a comprehensive range of community resources which most effectively meet the identified patient and family needs. Advocates for patient and family within coordinating agencies including Child Protective Services Takes leadership role in facilitation of interdisciplinary communication as it relates to patients and families experiencing complex psychosocial issues. Formally and informally educates patient/family and health care team members regarding the social work role, pertinent patient care issues, and population specific information. Serves as patient advocate and liaison with physicians, families and community to ensure continuity of care. Complies with the legal and regulatory laws, including mandated reporting requirements. Identifies and participates in Quality Improvement research and program evaluation initiatives that focus on improving health care services Documents patient information in EHR and/or other electronic system in accordance with program standards, department policies/procedures. May lead interdisciplinary team debriefings when needed due to loss or staff moral distress Represent and participate in unit or organizational DEI initiatives Opportunities to participate in educational presentations to internal groups including 'When A Baby Dies as well as outside organizations. Reasons for Social Work consults may include, but are not limited to, Complex psychosocial needs Mental health history or current diagnosis Substance use history or current use Positive toxicology screen for mom and/or baby Adoption and surrogacy Child Protection Services (CPS) involvement Fetal loss and Perinatal Palliative Care Birth to Minor/Adolescent (under 18 years old) Edinburgh score >10 AND any affirmative answer to question #10 (self harm) Qualifications Master's degree in Social Work with medical experience Current Minnesota Social Work Licensure at the LGSW Clinical Supervision provided EEO Statement EEO/AA Employer/Vet/Disabled: All qualified applicants will receive consideration without regard to any lawfully protected status
