Completes an initial psycho-social assessment including social work treatment plan. Provides individual, family or group counseling, complex discharge planning, bio-ethics case consultation, referrals to community agencies. Ensures continuity of care for those patients with a serious illness and maximizes the patient s and family s abilities to meet the continuing care needs within their social and financial framework. Qualifications: Required: + Graduation from an accredited graduate school of Social Work with an MSW (Master s in Social work) degree Certifications and Licenses Required: + State licensure required by the NJ State board of social work examiners. + Licensed social worker (LSW) or licensed clinical social worker (LCSW) depending upon years of social work experience and clinical course work. Scheduling Requirements: + Days + FT Essential Functions: + Understands stages of growth and development to assess each patient s age specific needs to provide age specific treatment and care. + Documents interventions in patient s chart and submits monthly statistical information to the department.
