Responsibilities What You’ll Do The Social Worker is responsible for patient advocacy, emergent counseling and education needs for patients and families, and effective discharge planning. The successful candidate will be able to:  Act as liaison between the patient and medical personnel Facilitate long term health and well-being through counseling, education, and brokering of needed community services and resources Develop and implement appropriate treatment plans for each patient Serve as an advocate for patients and their families in regard to decisions affecting plan of care Act as a resource for hospice and counseling services Provide appropriate end of life planning and care Ensure timely discharge planning Qualifications What You’ll Bring Master’s of Social Work degree required Previous hospital experience preferred  Experience in Acute Care preferred Must have a passion for excellence
