Social Worker (MSW)
Counsels, assesses, and assists individuals of all ages and families or groups who are affected by critical illness and post ICU recovery that may impair their ability to function normally. PRINCIPAL ACCOUNTABILITIES:                                                                 Provides psychosocial assessments to determine patient needs, makes appropriate referrals, provides counseling and supportive services to patients/families, makes referrals to the Division of Child Protection and Permanency (DCP&P) or Adult Protective Services.  Counseling for patients and families regarding adjustment to critical illness and its effects on individual and family coping.  Assist with identification and teaching of coping strategies.  Provide emotional support during goals of care discussions and assist with processing end of life decision making.  Provide anticipatory and acute grief support to patients and families.  Assist patients and families with navigating both the ICU and hospital system.  Coordinate and attend family meetings to provide communication and emotional support.  Provide assessment, preparation and counseling for children and teenagers visiting the ICU as well as guidance to their adult caregivers regarding how to support them.  Collaboration with Child Life Specialists as appropriate. Participate in relevant ICU and/or patient population committees.  Participate in relevant community collaboratives.  Contribute to program events, program development and relevant support groups.
