Job Overview:
Mental Health Professional, masters prepared to provide screening for trauma related social/emotional/psychological problems. This person will work in a primary care medical out-patient service at NBIMC and collaborate with medical staff on the identification and referral of patients who may have been exposed to traumatic experiences. This professional uses structured screening tools to identify specific mental health and substance abuse problems that may be associated with a history of trauma. This person will provide referral to appropriate mental health and/or substance abuse treatment providers and will be supervised by the Behavioral Health Department in conjunction with the Ambulatory Primary Care Department.

Completing initial psycho-social assessments on Intensive outpatient program patients. Providing psychotherapeutic counseling with individuals, family, or groups as needed.Providing patient-centered advocacy, collaborating with interdisciplinary teams, building and maintaining relationships with internal/external resources. Making referrals to community agencies and entitlement programs and continuing ongoing education as needed Ensures continuity of care for patients with complex post-acute needs and maximizes the patient's and family's abilities to meet the continuing needs within their social and financial framework,Understanding all stages of growth and development to assess each patient's age-specific needs to provide individual treatment and care, Responsible for reporting identified safety issues and ensuring appropriate follow-up is necessary.Benefits and Perks:
