**JOB SUMMARY** **BSW ROUND ROCK MEDICAL CENTER EXPANSION 2025** **\#rrexp24** Join us in an extraordinary opportunity to be at the forefront of our hospital's expansion in Round Rock, Texas. As we gear up to open our doors on January 11, 2025, you'll play a pivotal role in shaping the future of healthcare in our community. Our expansion, spanning 165,000 square feet, will introduce a state-of-the-art patient tower to our Round Rock hospital, tailored to accommodate the evolving needs of our rapidly growing population. Among its highlights, the tower will house an innovative 11-bed Neonatal Intensive Care Unit, alongside expanded capacities for crucial departments such as med/surg and labor and delivery The Social Worker MSW 1 provides patients and family members with the education and advice needed to cope with issues which arise from various emotional or physical problems as outlined by the Texas State Board of Social Worker Examiners scope of practice. Coordinates a variety of services including crisis intervention, bereavement, client advocacy, referrals, prevention, education, discharge planning and implementation, and psychosocial assessments. Partners in the development and implementation of performance improvement initiatives. **Location: Round Rock, TX** **Setting: Case Management** **Schedule: PT Every Sat/Sun 10 hr shifts** **ESSENTIAL FUNCTIONS OF THE ROLE** + Conducts and documents detailed psycho-social assessments to interpret social, environmental, and medical needs of the patient. + Employs advanced practice methods in the development, implementation and evaluation of treatment plans. Provides patient and family education and serves as a professional resource. + Provides or supervises plan of care interventions to include resource and referral help, crisis intervention, prevention, education, patient advocacy, bereavement, therapy, and counseling as appropriate. Partners with the healthcare team and involves the patient and family in the development and implementation of plans. + Serves as a liaison with the community resources and their staff to deploy smooth transition and placement of the patients within the assigned service line. + Partners with other healthcare team members to deploy the interdisciplinary care of patients and achievement of positive outcomes. + Participates in performance improvement initiatives to include data collection and review, development, and participation in continuing education programs. + Prepares and writes professional reports, letters, and case summaries.
