Responsible for social work evaluation of patient/family psych/social and environmental needs, as well as implementation of the social work treatment plan and documentation consistent with insurance guidelines. Work is evaluated by Home Health Manager through patient record documentation, verbal communication and direct observation.

Required Qualifications:

Master's Degree: MSW from a school of Social Work.   Within 30 days of hire: National Provider Identifier (NPI) number 1 year experience in a health care setting. Upon hire: Washington Advanced Social Worker License Or Upon hire: Washington Advanced Social Worker Associate License Or Upon hire: Washington Clinical Independent Social Worker License Or Upon hire: Washington Clinical Independent Social Worker Associate License 3 years Post Master's work experience, or prior experience in healthcare care setting. Preferred Qualifications:

2 years experience in a health care setting. Prior experience in home health/hospice care setting.
