Compassionate Caring is our Calling.** + Setting: Outpatient and Inpatient **Education** : + Master’s Degree in Social Work required. **Certifications/Experience:** + Minimum of one year experience in a hospital setting with at least three years’ experience in a relevant clinical role preferred. + Active licensure in the State of Indiana as a Licensed Social Worker or Licensed Clinical Social Worker required. A Social Worker (MSW) at Northwest Health plans, organizes, and implements discharge planning services to patients and families served by Northwest Health Hospital. This position exists to intervene with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. Services include, but are not limited to case identification, psychosocial assessment, care planning, social work consultation, patient/family education, short term therapy, and advocacy to facilitate continuity of care and access to community services. + This position provides case management leadership organization-wide, promoting team collaboration and continuous improvement of services. + Social Workers receive referrals for individuals from at-risk populations from care coordinators and interdisciplinary team members (including physicians and staff nurses), are involved with 100% of some specifically designated patient populations, and may intervene with patients identified as a result of case finding processes.
