Provides psychosocial assessment, crisis intervention, referrals and information to patients and families. Provides education/support services to patients, families and staff. Works collaboratively with the interdisciplinary team to effect positive patient and family outcomes and to ensure that resources are in place to facilitate the discharge plan. Serves as resource for less experienced social workers.

Required Qualifications:
Master's Degree Social Work from accredited university. Coursework/Training Courses or continuing education in Evidence-Based Intervention. 1 year Social work experience in a health care setting (outpatient/acute care or skilled nursing facility).Preferred Qualifications:
