Social Worker MSW

The Social Worker (SW), under direct and continuous supervision by an LICSW, provides biopsychosocial assessment and treatment/interventions/advocacy of patients and their support systems through discharge planning. The Social Worker incorporates concepts and theories associated with life span development, neurobiology, and behavioral change. The Social Worker will collaborate with the patient and support systems to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the Inter/Multi-Disciplinary Team (IDT).

Required Qualifications: 

Master's Degree in Social Work from a program accredited by the CSWE (Council on Social Work Education), or enrolled in a Master’s program to graduate within two months of hire Within 90 days of hire: Washington Clinical Independent Social Worker Associate License

Preferred Qualifications:

2 years Hospital / Healthcare experience
