Social Worker MSW - Home Health at Hospice and Home Care of Snohomish County

Schedule: Day Shift, 8:00 am – 4:30 pm, M-F
Location: Snohomish County

Job description:

This position is responsible for providing referral services to home care clients, families and caregivers, addressing issues such as adjustment to illness/prognosis, care system issues, community resource issues, and insurance/financial issues.  The MSW develops and implements a plan of care in collaboration with the Home Care team providing care to the patient.

Required qualifications:

Master's Degree in Social Work. Washington Advanced Social Worker License upon hire; or  Washington Advanced Social Worker Associate License upon hire; or  Washington Clinical Independent Social Worker License upon hire; or   Washington Clinical Independent Social Worker Associate License upon hire. National Provider Identifier (NPI) number within 30 days of hire. 1 year experience in a health care setting. This position requires that you obtain an NPI number within 30 days of starting in the position. The NPI process is free, and the application may take several weeks to process. Preferred qualifications:

2 years experience in a health care setting. Experience in a home health care setting.
