This position will be working with patients through the Napa Valley.

Job Description: Provides psychosocial assessment and delivery of counseling, social work and emotional support services to terminally-ill patients, and/or palliative services to patients and their families.  Works as a team member to identify problems and assist patient/family members in exploring realistic options, decision making and problem solving.

Required qualifications:

Master's Degree in Social work. 1 year of experience in a health care setting or related field.

Preferred qualifications:

Coursework/Training: Training in family counseling and case management. Social Worker License upon hire.
