Provides social work assessment, counseling, resource information/referral to neonate, pediatric, adolescent, adult and geriatric patients and families along with consultation and education to health care members.  Works collaboratively with interdisciplinary team to effect positive patient and family outcomes and to ensure that resources are in place to facilitate the discharge plan.

Required Qualifications:

Master's Degree in Social Work. 6 months Health care social work experience.
