**Job Description:**
The Clinical Social Worker utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide family and individual treatment as indicated. This position works collaboratively with patients, patient's significant other, families, healthcare providers, insurers, community resources, and all other involved parties.
**As an Oncology Social Worker you need to know how to:**
+ Ensures that patients receive timely and unimpeded access to the most appropriate, high quality, efficient, and cost-effective healthcare by working in a collaborative, supportive relation with the RN Case Manager, patient, family/significant other, and entire healthcare team.
+ The CM SW serves as an advocate to guide and refer the patient and family/significant other toward health promotion, illness prevention and patient centered goals.
+ Provides education to patient/family throughout stay to assist in adjustment to the illness and hospitalization to facilitate the development and acceptance of the discharge plans.
+ Performs initial and ongoing assessments of patients physical/function, emotional, social, spiritual, and financial needs of identified patients and implements crisis intervention and referral.
+ Serves as a resource to healthcare team throughout healthcare system in promoting evidence based medicine and provide education regarding healthcare and social resource systems to empower patient/family to access resources independently.
+ Confers with physician regarding psychosocial issues that may affect healthcare management.
+ Provides education and information to patients/families regarding Advance Care Planning.
+ Assists patients/families in developing and completing Advance Care Plan.
+ Provides information to patients/families regarding conservator ship and guardianship (throughout age-span), or facilitates presence of appropriate documentation in the medical record.
+ Continuously assess, develop, and implement discharge patient/family needs and work with appropriate healthcare provider to procure necessary services.
**Minimum Qualifications**
+ Masters Degree in Social Work or related field
+ Minimum of one year previous healthcare experience or minimum of three years in another setting
+ Current LCSW Licensure
+ Competent in medical terminology or successful completion of course at next offering
+ Ability to communicate effectively and diplomatically within a multi-functional team
**Preferred Qualifications:**
+ OSW certification is preferred

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$28.15 - $43.42
