Saint Alphonsus Medical Center in Baker City is seeking to hire a Part-Time **Social Worker.** Working under the direction of the Health Resource Center Manager you will deliver assessment, coordination, intervention and follow up planning for high risk patients presenting to the Emergency Department. Your goal will be to improve care outcomes with a focus on promoting wellness and efficient utilization of health services through developing a relationship with patients prior to their discharge. In addition, you will serve as a communication link between patients, nurses, physicians, families, significant others, ancillary departments, extended care facilities, community resources and insurance companies. You will conduct screenings and assessments that address barriers patients encountered while seeking routine primary care. Working with a patient resource nurse, a _Personal Pathway to Health_ plan will be developed that will guide and transition the patient into wellness and long-term health maintenance with a primary care provider. Finally, you will also collaborate with inpatient care coordination to assist in the prevention of re-admissions and increased compliance with Saint Alphonsus Health System value-based purchasing initiatives. **GENERAL REQUIREMENTS** **Education:** + MSW (Masters in Social Work) required **Licensure:** + LCSW or LMSW license required for Idaho and Oregon in accordance with the Non-Clinical Social Work Regulations **Experience:** + Minimum 2 years social work experience dealing with coordination, consultation, and facilitation of community resources specifically related to chronic conditions with co-morbidities + Experience in addiction services, mental health, or population health management desired **Our Commitment to Diversity and Inclusion** Trinity Health is one of the largest not-for-profit, Catholic healthcare systems in the nation.
