**Employment Type:** Full time **Shift:** Day Shift **Description:** Assesses patients and families and assists in discharge planning and coordinating care and patient transition post hospitalization. Adheres to Wheaton Franciscan Healthcare’s performance expectations and performs all defined services and other related duties in accordance with the mission, vision and values of Wheaton Franciscan Healthcare. **What you will Do:**   • Assesses prior living situation. • Assesses current situation and plans for discharge needs. • Assesses financial situation and discusses financial alternatives with patients and families. • Evaluates feasibility of services and resources (i.e., financial, psychosocial, willingness). • Provides documentation on chart of patient needs, resources and referrals **Hours/Schedule: **   80 hours biweekly (1.0 FTE) **Minimum Qualifications: **     + Bachelor’s degree in social work. + Current Licensed Social Worker in the state of Iowa or must obtain license in Social Work within 18 months of hire.
