(https://hr.uw.edu/benefits/wp-content/uploads/sites/3/2018/02/benefits-summary-classified-staff-greater-than-half-time-20230701\_a11y.pdf) **This recruitment offers a $5,000 Recruitment Incentive to eligible new employees joining UW Medicine** \#UWMRecruitmentIncentive The **UW MEDICAL CENTER - MONTLAKE - SOCIAL WORK DEPARTMENT** has an outstanding opportunity for a **SOCIAL WORKER to join the PEDIATRIC CARE CLINIC** . **WORK SCHEDULE** • 40 Hours/Week • Day Shift **POSITION HIGHLIGHTS** • Provide full range of direct care social work services (40% general social work care coordination and 60% clinical Behavioral Health (BHIP) model) to Pediatric patients ages 0-21 and their caregivers within a primary care clinic setting • Work in a Collaborative Care Model, provide screening for common Pediatric MH conditions with treatment interventions as clinically indicated • Serve as a liaison with medical providers to ensure increased adherence to both primary care and behavioral health treatment • Advocate for patients/caregivers' care coordination needs internal to UWMC, and external with schools and community agencies **PRIMARY JOB RESPONSIBILITIES** • Support and closely coordinate patient care as a generalist and behavioral health social worker within an interdisciplinary team of providers, RNs, residents, and psychiatry. • Provide evidence-based, mental health counseling in accordance with the BHIP model and Diagnostic Criteria (DC 0-5) manual, utilizing screening tools such as SMFQ, SCARED, Vanderbilt, PHQ9 and GAD7. • Demonstrate working knowledge and facilitate referrals to community resources, specifically related to Behavioral Health and Social Drivers of Health (SDOH) available to pediatric populations and their caregivers • Conduct comprehensive trauma-informed care, crisis/suicide intervention, parent-coaching, and psychoeducation within scope • Engage in regular case consultations with psychiatrist, SCH FAST, and social work team • Track referral volumes and outcomes to ensure engagement in services • Perform direct care in person, by phone and telehealth, with documentation and mandated reporting completed per UWMC and department procedures. Diagnoses and codes patient charts to ensure correct billing in accordance with UWM Compliance. • Provide work direction to Social Work Assistant(s) and Social Work student(s), as assigned to clinic. **REQUIRED POSITION QUALIFICATIONS** A Master of Social Work Degree from a program accredited by the Council on Social Work Education AND one year of full-time social work experience in a health care setting or equivalent.
