Social Worker (Program Coordinator)
Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review. Former EDRP participants ineligible to apply. Responsibilities Develop, maintain and adjust a psychosocial assessment of the veteran in collaboration with the veteran, the interdisciplinary treatment team, family members and significant others per ICMHR and Medical Center policy. Demonstrate an understanding of the range of treatment and skills needed for all veterans related to their physical limitations, psychological needs and age associated illnesses and conditions. Working within the interdisciplinary treatment team, will participate in developing a mutually agreed upon treatment plan with the veteran and family (when possible), which includes veteran-centered goals, objectives and interventions. Collaborate with other service providers in reassessing the veteran's needs. Collaborates with Supervisor and Executive leadership to maintain or modify guidelines and performance expectations for ICMHR team, which are clearly communicated through weekly team meetings and as needed. Responsible for establishing and maintaining effective working relationships with representatives i.e. points of contact for each discipline assigned or involved in each of the programs assigned to this position. Uses initiative and resourcefulness in deviating from traditional methods or trends and patterns to develop new methods, criteria, and proposed new policies. Practices at an advanced level and independently gathers relevant information to guide practice, including Social Work standards. Manages the Danville ICMHR team's daily activities, to include multidisciplinary staff assigned to ICMHR program. ICMHR SW Coordinator is given authority to make his/her own work assignments within the program area to which assigned in collaboration with Supervisor. Consults with the supervisor as needed and with colleagues to develop knowledge and apply agency policies and practices to new matters. His/her work is reviewed for technical adequacy and adherence to procedures and practices applicable to the subject. Responsible for collecting and submitting programmatic information to NEPEC, participate in quality improvement program to improve service delivery, and actively participates in performance improvement program in coordination with mental health Service Line. Performs a wide range of services and provides technical assistance to state, local , and private agencies and universities. Performs work related to the review, monitoring, analysis, planning and assessment of various programs. Responsible to actively participate in the program planning, development and evaluation in order to identify trends, opportunity for improvement, and make recommendations to Mental Health and Behavioral Health leadership. Enter all veteran/family contacts in the electronic record using appropriate formats and templates. Establish and maintain positive working relationships with employees, volunteers, consumers and stakeholders within the VA and outside community agencies. Attend all appropriate staff meetings (interdisciplinary treatment team meetings, Social Work staff meetings, Mental Health staff meetings, other committees as appropriate, etc.) and perform other duties as required. Collaborate in the performance improvement processes and address performance measures as required by the VA for their specific population and/or program. EDUCATION: Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. (TRANSCRIPTS REQUIRED) LICENSURE: Must be licensed or certified by a state to independently practice social work at the master's degree level. (COPY OF LICENSE REQUIRED)Exception: VHA may waive the licensure or certification requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure/certification examinations. This exception only applies at the GS-9 grade level. For the GS-11 grade level and above, the candidate must be licensed or certified. At the time of appointment, the supervisor, chief social work or social work executive will provide the unlicensed/uncertified social worker with the written requirements for licensure or certification, including the time by which the license or certification must be obtained and the consequences for not becoming licensed or certified by the deadline. GRADE DETERMINATIONS In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. GS-12 GRADE LEVEL EXPERIENCE: In addition to the basic requirements, you must possess one year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Examples of specialized experience at the GS-11 include, but are not limited to: assessing and documenting identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence; use of clinical social work skills; maintaining patient privacy and confidentiality per policies, handbooks or directives; acting as an advocate with appropriate community service providers and agencies when it serves the best interest of the patient and family members/caregiver; independently assessing the psychosocial functioning and needs of patients and their family members, identifying the patient's strengths, weaknesses, coping skills, and psychosocial acuity. AND Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice.
