Summary The VA Ann Arbor Healthcare System (VAAAHS) is a Complexity Level 1 b facility that is a major tertiary care referral center and provides care at Community-Based Outpatient Clinics in Toledo, Flint, Jackson, Livingston, Lenawee, Canton, Findley, and facilities at Packard and Green Roads. The incumbent serves as the Polytrauma / Traumatic Brain Injury (TBI) Coordinator for the Polytrauma Support Clinic Team (PSCT) & Post 9/11 M2VA Program TBI Case Manager (M2VA). Responsibilities Duties include, but are not limited to: The Polytrauma / TBI Coordinator Oversees all Polytrauma TBI activities and the coordination of care and services for active-duty service members and Veterans treated in the Polytrauma/TBI Clinic. The Polytrauma Coordinator coordinates services that include intake, assessment, and evaluation of both inpatient and outpatient referrals for Polytrauma/ TBI services for those active-duty service members and eligible Veterans. The Polytrauma / TBI Coordinator position works closely with the Physical Medicine and Rehabilitation (PM&R) and Post 9/11 M2VA Program (M2VA). Will coordinate rehabilitation services for Polytrauma TBI patients to ensure the best possible care in accordance with community and national VA standards. The Polytrauma Coordinator is responsible for recording, compiling, and completing reports related to TBI performance measures. The Polytrauma / TBI Coordinator is accountable for the coordination and management of services to clients and families in need of acute and chronic health management. Traumatic Brain Injury (TBI) Coordinator Coordinator of the TBI Polytrauma Support Assessment Team. The team comprises Physical Medicine and Rehabilitation (PM&R), Rehabilitation Therapists, Mental Health and Medical Providers, a Registered Nurse, and Patient Advocate. As the facilitator of this interdisciplinary team, the TBI Coordinator ensures that clinical reminders, patient evaluations, and referrals are facilitated. In addition, performs social work assessments and leads interdisciplinary team conferences. Post 9/11 M2VA Program TBI Case Manager Provides comprehensive social work case management to respective Post 9/11 transitioning and established active-duty service members and Veterans (SM/Vs) and their families across the continuum of care and during the various phases of readjustment to civilian life. Independently provides an array of social work interventions including high-risk screening, psychosocial assessments, treatment planning, resource education and referral, and case management services. These services are provided in healthcare settings, both inpatient and ambulatory. Ensure that the transition of care of Veterans and Active-Duty Service members, from the DoD Military Treatment Facilities (MTF) and Warrior Transition Units (WTU) to the Veterans Health Administration (VHA) is safe and continuous. The M2VA Case Manager supports the Veteran, the Veteran's family, and the Veteran's Caregiver's and coordinates health needs across agencies, sites, and unique episodes of care to optimize service delivery, improve quality of care, and enhance both patient experiences and outcomes. In addition to the clinical and case management responsibilities, assists with the public affairs component of the M2VA Program's national mission through outreach, e.g. Yellow Ribbon Reintegration Program (YRRP), Post Deployment Health Reassessment Programs (PDHRA) with local military units returning from combat missions. Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice.
