To provide individualized psychotherapy, family education, and therapeutic group sessions to acute inpatient gero-psych patients aged 60 and over. Supports discharge planning and utilization review services within a multidisciplinary treatment team in the Senior Behavioral Health Unit.
Minimum Qualifications:
Master of Social Work (MSW) degree required, License Certified Social Worker (LCSW) in the state of Oregon preferred.Minimum 1-year internship experience, preferably in a hospital or health care setting.Must have the ability to work efficiently, responsibly, and independently.Excellent time management skills required.Must possess strong interpersonal skills with a high sensitivity to the needs of others, including the ability to communicate effectively with staff, administrators, patients, families, physicians, volunteers, and other customers.Nonviolent Crisis Intervention Training within ninety days of hire.Essential Job Functions:
Complete Psychosocial Assessment, Mental Status Exam, and Alcohol Use Disorders. Identification Test within 72 hours of admission.Coordinate and contribute towards individual and group therapy activities according to an established program schedule.Coordinate discharge planning in conjunction with the multidisciplinary team.Offer family education as needed to complete discharge coordination.Participate in daily treatment team.Complete utilization review in an organized and timely manner.Knowledge of Community Resources and strong networkingAssumes responsibility for meeting annual in-service requirements and ongoing personal developmentKnowledge, Skills, and Abilities:
