Comunilife is looking for an experienced Senior Social Worker to fill its open position for their Safe Haven I Program in the Bronx. The Senior Social Worker will conduct psychiatric and other assessments, as well as develop and implement person-centered treatment plans, and supportive services.  Services are delivered through evidence-based client treatment practices. The positions offer a generous benefit package including health insurance, dental, vision, dependent care, paid time off, 401(k), tuition assistance program and more.    Essential Duties Assess clients’ substance abuse, health, mental health, and other care needs; recommend internal and external referrals;  Provide individual and group counseling; Prepare psycho-socials; Facilitate psycho-education groups in relapse prevention, health literacy, and navigating mental health and substance abuse systems, etc. for residents their family and/or support networks; Provide Comprehensive Case Management services; Assist clients in improving and maintaining self-care, interpersonal, and vocational skills  Assist clients in achieving housing stability  Manage discharge planning; engage and support clients 90 days post discharge; Manage client documentation, provide program narrative and/or statistical reports as needed; Participate in ongoing QI and program evaluation activities
