Summary:

Supervises the daily staff activities of Social Worker staff in addition to their normal social worker responsibilities. The social worker provides patient education and emotional support, is responsible for referrals to community agencies, coordinates patient care with other team members, and assists with concrete services.

Responsibilities:

Assesses quality of services delivered and facilitates staff development programs. Ensures staff compliance with departmental and organizational policies, procedures, and protocols.Performs staff responsibilities to fulfill required service levels. Leads the handling and resolution of complex issues and complaints.Responsible for program's patient's social needs including discharge planning for inpatient and outpatient. Responsible for evaluations to include psychosocial assessments in order to determine appropriate candidacy and continue to work with patient and family. Provides ongoing assistance with social needs to all patients. Responsible for the presentation of patients at selection committee as required by CMS where appropriate. In addition, oversees and is a resource to other specialty social workers.Provides direction, orientation, training, coaching, and mentoring to staff. Performs or assists with performance evaluations and disciplinary actions.

Required Work Experience: Five (5) years' experience. Other Information: Active participation in social work professional organizations preferred.
