Overview

The Medical Social Worker provides social work services to patients in their place of residence, in accordance with the physician orders and plan of care under the supervision of the Clinical Manager or designee.  

Responsibilities Understands and adheres to established company policies and procedures Participates in the coordination of company services with community resources and services, serving as liaison between patients or family/caregivers and community resources Maintains collaborative relationship with organization personnel to support patient care Observes confidentiality and safeguard patient information Serves as a resource for employees, patients, physicians, and other allied health providers Develops a cooperative relationship and communicate effectively and professionally with healthcare providers Maintains current knowledge of Medicare Conditions of Participation for social work services Participates in in-services and/or continuing education programs Participates in patient care conferences Maintains contact with community support groups and provide professional expertise Plans and coordinates all social services within the company Documents all patient/family services provided as required by company policy Assists physician and/or other care team members in understanding the significant social and emotional factors related to health problems Participates in the development of the plan of care Prepares clinical and progress notes in accordance with company policy Participates with patient discharge planning Supervision of assistant personnel when required by law Other duties as assigned and within scope of practice Qualifications Master’s or Doctoral degree from a school of social work accredited by the Council on Social Work EducationOne year of social work experience in a healthcare setting required, three to five years preferred One year in home health care preferredValid license from the state practicing in Current CPR certificationBasic clinical skills Professional appearance and demeanor General knowledge of computers and the ability to learn industry specific software applications Excellent oral, written, and interpersonal communication skills Excellent organizational and time management skills with a keen attention to details Ability to work effectively in a team environment Ability to develop critical thinking skills, research situations, solve complex problems and deal with a variety of issues Ability to learn and complete various educational goals during orientation About our Line of Business Hospice Home Care focuses on providing hospice care to local patients and their families living in Arkansas.
