They meet with students individually and/or in group sessions. Special Education Teacher/ Transition Study Instructor will be trained in either the SDLMI-Transition or SDLMI-Transition with Mentoring programs and then be responsible for implementing the program during the 2024/2025 and 2025/2026 school years. Special Education Teacher/Transition Study Instructor provides program services to Special Education students at Canyons School District. This position 2 year position as it is funded by a grant from 2024-2026. ESSENTIAL FUNCTIONS + The Special Education Teacher/Transition Study Instructor will be trained in either the SDLMI- Transition or SDLMI Transition with Mentoring programs and then be responsible for implementing the program in the Canyons School District during the 2024/2025 and 2025/26 school years. Special Education Teacher/Transition Study Instructor provides program services to students at Canyons School District. In addition, the Special Education Teacher/Transition Study Instructor agrees to act consistently in accordance with the philosophy, policies, rules and regulations of Canyons School District now in effect and coming into effect. + Attend in-person and virtual trainings during Fall 2024 and ongoing technical assistance throughout the delivery of the program. + Teach students with disabilities the core content area(s) outlined in the USOE’s Core Curriculum. Attend IEP meetings. + Maintain student records as required by IDEA. + Assure confidentiality of all students’ information. + Develop lesson plans and instructional materials and provide individualized and small group instruction in order to adapt the curriculum to the needs of the child. + Establish and maintain standards of pupil behavior needed to achieve a functional learning atmosphere in the classroom. + Maintains appropriate records + Communicate with parents through conferences, telephone calls, e-mails, student planners, school website, and other means, to discuss the pupil’s progress. + Identify pupil needs and cooperate with other professional staff members + Provide and maintain a safe and supportive environment in the classroom and on the school campus. + Maintain professional competence through in-service education activities provided by the school district and self-selected professional growth activities. Demonstrate professional ethics as outlined by the Utah Professional Practices Advisory Commission. + Be a positive role model. + Participate in faculty meetings as required. + Participate in team meetings. + Function as a member of the special education team. + Administer appropriate pupil attendance policies and procedures. + Participate in appropriate educator evaluation procedures as per district guidelines. + Participate in faculty committees and the sponsorship of pupil activities. + Model professional and ethical standards when dealing with students, parents, peers, and community. + Is responsible for understanding and adhering to laws, district policies, and procedures. + Requires strong oral and written communication skills with students, parents, and staff. + Requires effective problem solving, organizational, multi-tasking, and time management skills. + Requires the ability to implement effective teaching strategies. + Requires the ability to establish and maintain a positive learning environment. + Requires the ability to calmly handle classroom management issues. + Requires the ability to work as a member of a team as well as the ability to work independentlywith little direct supervision. + Knowledge of State Core Curriculum and Board of Education adopted guidelines. + Deliver small group instruction and 1:1 mentoring following program materials during the Spring and summer 2025, Fall 2025, and Spring 2026 Semesters. + Communicate with students’ families to support student-family transition planning activities during the Spring and summer 2025, Fall 2025, and Spring 2026 Semesters. + Communicates the goals of the transition study program to education stakeholders. + Maintains current and appropriate program data for education stakeholders. + Uses at least 95% of time providing direct services through the curriculum (guidance curriculum activities) and Individual Student Planning and most remaining time in program management, system support and accountability. + Accurately and appropriately interprets and utilizes student data. + Collaborates with parents/guardians and educators to assist students with educational and career planning. + Consults and collaborates effectively with parents/guardians, teachers, administrators and other educational/community resources regarding students with identified concerns and needs. + Assists teachers, parents/guardians and other stakeholders in interpreting and understanding + Uses available technology resources to enhance the impact of the program. + Technologies such as websites, interest surveys, career explorations, and links to community resources. + Be a positive role model. + Participate in faculty meetings as required.Uses available technology resources to enhance the impact of the program. + Technologies such as websites, interest surveys, career explorations, and links to community resources. + Work with the administration to create, coordinate and implements student orientation programs forth incoming students. + Monitor students’ progress towards graduation, identifies credit deficiencies, and facilitates posts secondary plans. + Conduct parent education classes in cooperation with the special education department requirements. + Predictable and reliable attendance. During times of state or district emergency, all employees may be required to perform additional duties that are not required on a normal basis in support of educational and operational priorities. e.g. Performs general cleaning and sanitizing of work areas, provides remote assistance to students and staff, and any other reasonable duty as need.
