A Specialized Medical Care Manager contributes to AHF by using a strengths-based approach to identify why each client is out of care, focused on re-engaging clients at risk of falling out of care and providing customized interventions that ensure continuous engagement. The Specialized Medical Care Manager (SMCM) will provide education and assistance to Ryan White with eligibility. The SMCM will work with the client to identify and utilize the client’s current personal strengths to address and overcome barriers preventing the client from continuously engaging in care and treatment. By allowing clients to prioritize their own service needs, the SMCM empowers clients to take charge of their care and build a plan that feels best suited to his/her individual needs. Working directly within the Healthcare Center, you will meet with clients and complete Needs Assessments, develop Enhanced Care Plans, and provide ongoing education to clients so that they understand their diagnoses and their medications. Once client re-engagement is established, the SMCM will ensure a smooth transition to traditional MCM for ongoing support. This position will require the SMCM to attend the monthly Outreach Workforce Committee Meetings conducted by the Ryan White Program.


A Specialized Medical Care Manager enhances the reputation of AHF by utilizing your case management experience to meet our clients where they are, identify strengths and opportunities for growth, and assist them with realistic goal setting. With your collaboration and engagement skills, you will support Returning to Care (RTC) clients. The SMCM will work closely with the Outreach Worker as clients are located and prepared to re-engage agency services and medical care. The SMCM will heavily focus on building a Client’s support system (family/friend/partner/spouse). By establishing a Needs Assessment and an Enhanced Care Plan, your work will directly contribute to AHF’s success in meeting the deliverables of our Ryan White/National HIV/AIDS Strategy (NHAS) supportive contract. You will customize services by developing an Enhanced Care Plan (ECP) with the client to re-align the client’s perception/prior experience of service usefulness and accessibility barriers, allowing the client to set service priority. The SMCM will monitor to assess the efficacy of the ECP, to include homr/field visits Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures and the Department of Health and Human Services.

The Specialized Medical Care Manager will work as a part of a close-knit team to support both clients who are returning to care and the Outreach staff by using a compassionate approach to keep the clients in care. The SMCM will identify emerging barriers that may prevent each client from maintaining participation in care and treatment. The SMCM will maintain knowledge of community resources and methods to access services. The SMCM will work together with the client towards viral suppression, better health outcomes, and retention in care.


Licenses and Certifications:

- Bachelor’s or Master’s Degree in Social Science, Social Work, or Human Services
- Able to travel within the service area 40% or more
- A minimum of 2 years of Case Management experience is preferred
- Knowledge of Ryan White Services is preferable
