Specialty Care Coordinator - Women's Health
Job Overview:

The responsibility includes coordination and oversight of the ongoing Multidiscipliary Clinics including collaboration with multispecialties, patients and caregivers. Responibilities also include coordination and management of prescribed oral medications including prior authorizations, patient assistance, monitoring of oral compliance, etc.

Job Requirements:

Graduate of an approved technical, professional, or vocational program in Healthcare; i.e. Medical Assisting, Surgical Technology, Health Administration, or similar
Front office and back office operations of a physician practice 
EMR 
Microsoft Office
Participation in a National Professiona Organization is desired
3-4 years experience Physicians Practice Setting

Job Responsibilities:

Provides for the navigation of care to the identified population by serving as the point of contact for referring physicians, patients, and families. Facilitates referrals with the multispecialty team. Coordinates patient infromation necessary for an efficient, successful referral, and patient care.
Monitors and tracks trends on referrals to foster team awareness and physicians' relationships.
Coordinates and manages oral medications as prescribed. Manges the workque to ensure insurance authorization and verification. Acts as a specialty liason between the patient/caregiver and specialty pharmacy. 
Follows up on patient complaince with oral chemotherapy.
Coordinates FMLA paperwork.
