Ø Establish specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance. Ø Provide in person, clinic visits as needed at the initiation of care and throughout transitions of care throughout all of our hematology/oncology clinic sites, as needed. Ø Formulating custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care. Ø Providing disease and treatment specific education tailored to meet patient’s individual goals of care. Ø Onsite rounding throughout treatment plan implementation as needed to provide needed in person patient evaluation. Ø Provide direct, hands-on nursing care, as needed. Ø Assist patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance. Assisting patients with social services needs by identifying resources in the community and referring to these programs.
