SNN now facilitate communication among obstetricians, maternal-fetal medicine specialists, neonatologists, genetic counselors, acute care teams, and subspecialists.
The program addresses the unique healthcare needs of expectant mothers and their unborn infants with complex medical conditions. It ensures comprehensive and coordinated care throughout the perinatal period, improves maternal and fetal outcomes, enhances patient experiences, and promotes overall well-being for families facing complex perinatal challenges.
As a Specialty Nurse Navigator, you will be responsible for:
· Formulating custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care.
· Establishes specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance.
· Assists patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance. Identifies and aligns resources that impact high-risk care.
· Coordinates services on behalf of the high-risk patients.
· Work in collaboration with the Healthcare team to track and review data for research projects as well as resource utilization related to relevant specialty and make recommendations as needed.

Required Education/Experience/Specialized Skills:
· BSN plus 3 years recent clinical nursing experience in a variety of clinical settings.
Required Certification/Registration:
· CA RN License
· BLS if working in a clinical setting.
Preferred Education/Experience/Specialized Skills/Certification:
· MSN
· Two years experience managing care of relevant specialized patient population (Labor and Delivery, Post Partum or Perinatology)
Required Education/Experience/Specialized Skills:
