Responsibilities
The Therapist provides patient care, including evaluating the patient for treatment, providing and directing administration of the care plan upon the referral from the physician. Consults with other health care disciplines to coordinate therapeutic programs and discharge planning for individual patients. Instructs patients and caregivers in home program exercises and functional activities. Assists with patient program management, while delivering quality patient services.
Completes documentation of treatment minutes and enters electronic charges accurately and timely.Participates in continuing education and develops advanced skills that enhance and align with department, clinical and business development.Completes general and specialty competencies appropriate to clinical area for patient care management (General, ICU, FIM, DBS, Lung Transplant, Behavior management, MBS, FEES, etc).Reviews and interprets all pertinent data (medical, psychosocial, historical, cultural) related to the condition(s) during the evaluation, treatment, reassessment and discharge planning.Selects and performs appropriate standardized clinical tests and measures to determine appropriate treatment goals and plan that is based on data analysis and evidence for practice with clear documentation justifying the need forAchieves annual department goal for patient satisfaction.Advances new programs and services and adopts new technology (e.g. FEES, Physiotech, Alexicom, Myomo,).Provides ongoing communication and education, both written and verbal to all parties involved in the patient care to ensure quality of care and achievement of patient outcomes.Follows all clinical and operations policies and procedures related to patient safety.Qualifications
Minimum:
Internship or rotation in appropriate clinical setting required.Graduate of an accredited Speech/Language Pathology program or related field required. Computer skills required. Arizona License in professional area. American Heart Association BLS required.Preferred:
2 years progressive experience in clinical area performing patient clinical care preferred. Master's or Doctorate degree in appropriate clinical area preferred. Prior use of electronic health record preferred.
