Speech Language Pathologist - Home Health

In response to a physician's referral, provides speech/language/cognitive/oral-pharyngeal functioning evaluations; develops, coordinates, and executes effective SLP services; and makes appropriate referrals and coordinates care.  Will provide direct patient care to individuals of all ages (neonatal to geriatric) as their expertise allows and caseload demands in the home setting.  Coordinates and provides patient/family education.  Direct supervision will be provided by the Clinical Supervisor and the Therapy Coordinator.  Participates in educational programs and performs non-patient care responsibilities as assigned.  Participates in program development and in the QA/CQI program.  Consults and coordinates with interdisciplinary team in providing comprehensive treatment.  Pursues continuing education and assists in the marketing of SLP services.

 

Qualifications: Current temporary or permanent S.C. license issued by the S.C. Board of Examiners in Speech-Language Pathology and Audiology, and an approved Clinical Fellowship Year Plan or a Certificate of Clinical Competence from the American Speech-Language-Hearing Association.
