This is a part time (24 hours/week) position working on weekdays during day shift and supporting our Laurelhurst and Irvington/Marie Smith sites.
This position is responsible for Speech-Language Pathology(SLP) evaluations and implementation of the Speech Therapy plan of care in accordance with PACE policies. The Speech-Language Pathologist works with the Interdisciplinary Team and is responsible to communicate and coordinate with the IDT as well as multiple outside providers, caregivers and family members. The SLP functions independently and may see participants in multiple settings including the PACE center, participant homes, Adult Family Homes, Assisted Living Facilities and nursing homes.
