Speech Language Pathologist-PV

The Speech Language Pathologist assesses, plans, develops and provides the patient or responsible caregiver a home health plan of care for speech language pathology and identifies patient needs for other disciplines and services.  Coordinates and evaluates the plan of care with all other disciplines and physicians for an assigned caseload.  Instructs the patient or responsible caregiver on the projected care plan and expected outcomes as directed by the physician's treatment plan.

 This position covers Team 1: Bethpage, Great Neck, Manhasset, Roslyn, and surrounding areas. 

Requirements:

Licensed and current registration to practice as a Speech Language Pathologist in New York State required. Minimum of one year post-licensure experience in rehabilitation with adult speech/language impairment populations. Good computer skills - Electronic Medical Record experience required.

 The pay for this position ranges from $75.00-$80.00 per visit.
