Who We Are:
Concierge Nursing Referral Services is an established, premier nursing referral service branch of TheKey that offers placement assistance for nurses (RN and LVN) and therapists (PT, OT, etc.) with distinguished VIP clientele in their own homes.
We partner with experienced and compassionate clinicians to provide private-duty clinical services, assisting clients with hourly shifts (generally 8-12hrs) and/or visits (up to 1 hour) on a per-diem or long-term basis. We are not Home Health and do not accept insurance. All clients pay privately for services and expect the very best quality care from our clinicians. Our clients are located throughout Southern California, up to the Bay area, going as far North as Napa County.
What We Offer:
Competitive rates starting at $200 per visit (varies by client, negotiated prior to starting case) paid weekly.Flexibility. As a 1099 independent contractor, you determine your schedule, and which clients/cases you'll accept.1:1 In-Home Patient Care. Finally get out of chaotic, clinical environments and practice care in the comfort of patient homes ResponsibilitiesEssential Functions:
Provide a comprehensive assessment to develop and implement an individualized programProvide direct patient care to maximize a patient's progress toward achieving functional goalsWork within a multi-disciplinary environmentPractice effective communication to consistently review patients' functional needsAdjust care plans to ensure optimal patient outcomesCoordinate services by participating in case conferences, case management, team meetings, and staff meetings as indicatedMeet all documentation requirements according to referral agency policy and procedurePrompt and effective communication with appropriate parties (doctor, family, office, etc.) when necessaryAssist client and/or family with training as needed QualificationsRequired Skills, Education and Certifications:
Speech Language Pathologist (SLP) in the state of California (required)2 years of therapy experience strongly preferredMust be able to work independently with little to no supervision or trainingPrevious home care / private duty experience Must be reliable, dependable, and on timeBasic computer skills as well as overall technical savviness Proof of eligibility to work in the U.S.
