SUMMARY

ATTRIBUTES
Minimum of nine (9) months of supervised work experience (completion of CFY), or participation in supervised clinical fellowship, for a minimum of 36 weeks under a NYS licensed Speech Pathologist, followed by the NYS licensure examination and achievement of NYS licenseOne (1) year experience with frail elderly population preferredCompassionate, warm and patient focusedExcellent organization and planning skillsRESPONSIBILITIES
Evaluation & Planning. Perform a complete initial evaluation of referred patient; develop and implement treatment plans, including appropriate short- and long-term goals; involve the patient, family and staff in the implementation of the treatment program; re-evaluate patient functional status progressively; advance patient toward maximal performance and functional independenceDocumentation. Maintain a complete documentation of patient evaluations, treatment, progress and discharge status; send required information to appropriate professionalsCollaboration. Actively participate in rehabilitation team efforts to meet patient needs, including counseling and teaching patient and family, discussing changes in treatment programs with MDs, coordinating healthcare plans with other involved professionals and assisting in discharge planning as appropriateEDUCATION:

14621The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
