JourneyCare Home Health, formally Addus Home Health, was founded over 40 years ago. We started out as a home cleaning service and then began providing chore services to Chicago’s at-risk elderly population through a contract with the city. What You’ll Do:

PRN schedule only.Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.Identifies functional speech defects and establishes a plan of care to improve patient’s function.Administer diagnostic speech and language evaluations and some basic, informal audio logical assessment in accordance with the plan of careRecommends communication devices/aids as indicated.Prepare a report of each client’s diagnostic evaluation and make recommendationsAdvise, consult and teach appropriate procedures and supportive activities to clients, family members, and other agency staffProvide direct speech service to clients under a provider’s plan of care. Records and reports to the provider the client’s response to therapy

Qualifications:

Possess certification of clinical competence in speech from the American Speech Language Hearing Association Licensed as a speech therapist in the state in which practicingAt least one (1) year of acute care setting or equivalent experience Possess knowledge of principles and practices of speech therapyComplies and maintains current CPR certification.Ability to make home visits and provide nursing care in the home environmentMust have reliable transportation, current driver's license, and appropriate automobile insurance
