All while understanding that **together** we are even better. **Schedule:** Part Time **Shift** : Day Shift Schedule Monday- Friday **Location:** Covering Buncombe, Henderson, Polk, Transylvania with future growth into Rutherford, McDowell, Madison, Yancy and Haywood Counties. **The role you’ll contribute:** The Home Health ST Visiting therapist, under the general supervision of the Case Manager, provides therapy to assigned patients based on individual patient needs. Assesses, plans, implements, evaluates total patient care and coordinates patient/family teaching. Assures compliance with therapy standards, federal, state, and local government regulations, and company clinical policies. Provides goal directed care leading to specific outcomes. May be on call for after-hours work. Coordinates and directs the home care patient's care based on individual patient needs. **The value you’ll bring to the team:** + Performs an evaluation and utilizes the findings to determine diagnosis (for SLP services), prognosis and plan of care in consultation with the physician. Determines specific interventions, duration and frequency needed to achieve anticipated goals and outcomes. + Observes and records activities and findings in the clinical record and reports to the physician and or primary Case Manager the patient’s reaction to treatment and any changes in patient’s condition, or when there are deviations from the plan of care. + Instructs the patient and caregiver regarding the patient’s speech language improvement program including adaptive equipment and the home program. + Instructs other health team personnel including, when appropriate, home health aides and caregivers in certain phases of speech therapy with which they may work with the patient. **The expertise and experiences you’ll need to succeed** **:** **Minimum qualifications:** + Master's Degree in Speech Therapy + 1 year relevant clinical Speech therapy experience + Speech Therapist License in State of Practice + Active American Heart Association BLS + Valid in state Driver’s License with current car insurance **Preferred qualifications:** + Recent, relevant experience in a Medicare-certified home health agency as a case-manager + Certificate for OASIS Specialist – Clinical + Home Health Case-Manager Certification This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
