Position: SPOA Housing Case Manager
Division: Mental Health Housing Services
Program: Intake Department
General Overview: Short term Case Management for a caseload of residential housing Residents/Tenants needing enhanced services to support admission into CHOICES Housing Services or to transition to an alternate level of care.
Qualifications: Preferred Bachelor’s Degree in Social Work, Human Services, Community Mental Health or related field. One year of human services, preferably Adult Mental Health. Must have clean and valid NYS Drivers License. Must use personal vehicle with mileage reimbursement in performance of job function.
Hours: Full Time
Salary Range: $38,355 - $43,000, annually
