Sponsored Residential Host Family

We are currently seeking a Sponsored Residential Host Family to join our Intellectual and Developmental Disabilities Services (IDD) division in Kilmarnock, VA.

 

A Sponsored Residential Provider offers a supportive, family-like home environment for individuals with intellectual, developmental, or other disabilities, fostering independence, community integration, and personal growth. The provider is responsible for delivering care tailored to the individual’s unique needs, guided by an individualized service plan. This role involves promoting skill-building, ensuring safety and well-being, and facilitating access to social, recreational, and community-based opportunities.

 

Pay Range $50,000 to $120,000

 

Job summary:

Provide a safe, clean and pleasant living environment for the person living in their home. Ensure compliance to state, local, federal, and contractual requirements. Ensure that the individual’s hygiene needs are cared for and that they have their own personal hygiene items such as a hairbrush, comb, toothbrush and paste, deodorant, cosmetics and shaving equipment. Provide training and supervision as needed. Ensure that the individual wears clean and neat clothing which is age and culturally appropriate, and which is appropriate for the weather. Assist the individual in selection and purchase of clothing, and other personal possessions.  Maintain records of expenditures. Ensure that the individual receives three meals per day and snacks which are properly stored and prepared, and which are nutritious, including, one item from each of the following food groups: dairy, fruit, and vegetable.  Grain should be included in at least one meal each day.  Provide training in simple meal preparation, cleanup and food shopping. Provide adult supervision at all hours when the individual is in the home (exceptions will only be made if there is a current plan stating the individual can maintain identified periods of time at home alone). Ensure that the physical health of the individual is protected by providing transportation and supervision to physicians, dentists and other health professionals when needed. Assist individual in following doctor’s orders, including medication as prescribed, special diets, etc. Participate in the development of the individual’s plan. Provide training to the individual in areas identified as SRP’s responsibility in the plan.  Maintain required records such as documentation of fire drills and monthly reports, including weekly observations, medication sheets and doctor report forms. If applicable, maintain regular and appropriate contact with employment or day services, including visits when needed. Work cooperatively with the individual’s natural family or advocate in cooperation with the individual’s team. This may include arranging for visits with family, ensuring that the individual has regular phone contact with family as desired, providing appropriate information to the family on the individual’s progress and any other related duties as determined by the individual and team. Include the individual as a participant in family recreation and leisure activities on a frequent basis and assist the individual in planning physical and social activities out of the home on a regular basis or as determined by the plan. Ensure the confidentiality of the individual’s records, names and identities, health information, etc., except as disclosure is permitted or required by law. Permit Merakey staff personal access to the individual at reasonable times and at any time Merakey personnel necessary for emergency purposes and/or unscheduled supervisions as required by regulation. Permit visits to the home by Merakey staff, licensing audit and others as required by the agency. Share with Merakey staff on a regular basis all important decisions affecting the welfare of the individual and knowledge of the individual’s adjustment in the family home, employment or day services and in the community. Notify the agency at least 30 days in advance of a change in address or telephone number. Report any change in Sponsored Residential arrangements, including the discussion of plans with the Supports Coordination and Merakey staff, before boarding or lodging other children or adults in the home, including family members. Notify the agency at least 30 days in advance before taking the individual for extended trips in time or distance and notify the Merakey staff about any substitute care arrangements. Substitute providers are responsible for meeting licensing requirements outlined in Merakey Sponsored Residential Substitute Provider Checklist. Attend all provider family group meetings for the purpose of training and support. Attend/complete required trainings
